Chronic Medical Condition Questionnaire

In situations where an employee indicates that a chronic medical condition is impacting their ability to attend work regularly, a Chronic Medical Questionnaire may be provided to the employee to share with their treating physician. This questionnaire focuses on an employee’s functional abilities and limitations, as well as how a chronic medical condition may affect their ability to attend work regularly.
