In situations where an employee indicates that a chronic medical condition is impacting their ability to attend work regularly, a Chronic Medical Questionnaire may be provided to the employee to share with their treating physician. This questionnaire focuses on an employee’s functional abilities and limitations, as well as how a chronic medical condition may affect their ability to attend work regularly.

This guide provides leaders with a clear, structured framework for attendance-related discussions. It includes guidance on how to approach conversations in a respectful, supportive, and consistent manner. The goal is to build confidence in having timely attendance conversations while reinforcing the supportive intent of the AS program.
